Request A Transcript
In accordance with the Family Education Rights and Privacy Act, transcript requests must be made in writing and include the student’s signature. Electronic signatures and requests received
via phone or email cannot be processed. All financial holds must be resolved prior to requesting a transcript.
Please download and print* a Request for Transcript (pdf) form. Complete & sign, then mail, fax or hand deliver request form(s) to:

You may also send a letter, instead of a form, to request a transcript. Please include the following information and send to the address or
fax number shown above:
- Name of student (printed)
- Other name(s) previously or currently used (if different)
- CNU ID Number (if available)
- And/OR Social Security Number (optional)
- Date of birth
- Your return address
- Phone number where you can be reached
- Years of attendance (especially if before Fall 1979)
- Level of classes enrolled in (undergraduate, graduate, or both)
- Complete name and address of person, school, or organization where transcript is to be sent
- Number of copies
- Signature of Student (required)
Transcripts are free of charge and will be sent out in turn as requests are received. Please allow three to five business days for processing. At semester end (Dec/Jan or May/Jun), allow at least seven to ten business days for processing.
Transcripts may also be picked up by the student upon request, with picture ID, at the Office of the Registrar three to five business days after receipt of a request. We are located on the second floor of the Administration Building on campus. Transcripts issued to or mailed directly to students will be stamped "Issued to Student."
