Who May Use Our Services

  • All students*, staff and faculty
  • *All students under the age of 18 must have a signed Parent / Guardian Consent for Minors form (Get Adobe Reader for PDF, new window) on file at the UHWS in order to be seen in the office of UHWS.

Services Offered
Payment is due at time of service. UHWS accepts cash, checks made payable to Riverside Business Health Services (RBHS) and Captain's Card flex points.                    *****   Please note that no credit or debit cards accepted.

Allergy Shots
Because of safety and liability issues, no allergy shots are given in the office. Students are urged to contact their insurance company to find out which medical facilities they are authorized to receive them in. There are several local family physicians who can administer allergy shots, after an initial evaluation visit.  Please contact the office of UHWS for further assistance and more detailed information.
