Hours may change based on demand and/or circumstances beyond our control. Please call (757) 594-7661 to confirm current hours and to schedule an appointment.    Appointments cannot be made by email.

First appointment in Health & Wellness Services?

New Patients:  Please arrive about 10-15 minutes early to fill out the necessary paperwork or:
Save time and fill out the necessary forms before you arrive  >>>>>>>>   ( MS Word, be sure to print all pages!)

• If you have been here before, and
• Have changed your local address/telephone number, or
• Have had any changes to your medical health/status,
• please let us know by filling out and bringing in a Patient Information Update Form (MS Word)

Visits with a Nurse or Physician (when available) are by appointment only

Please be respectful of others and be on time for your appointment.

 In order to insure that all patients are seen at their scheduled time, being 15 or more minuets late for your appointment will necessitate  rescheduling.

Appointment Fees:

Nurse Practitioner (FNP) and Physicians   Fee:  $20.00  Cash, Check or Flex Card

Registered Nurse (RN) - You may see the Registered Nurse by appointment.  Fee: No fee
                  Lab work is an additional cost depending on what is required.  Labs can be billed to insurance.
