If you choose, you may send a letter instead of our form to request a transcript. Please include the following information and send to the above address or fax number:
* Name of student (printed)
* Signature of student (required)
* Other name(s) previously or currently used (if different)
* CNU ID number (if known)
* Social Security Number (optional, for historical records only)
* Date of birth
* Your return address
* Phone number where you can be reached
* Years of attendance (especially if before fall 1979)
* Level of classes enrolled (undergraduate, graduate, or both)
* Complete name and address of person, school, or organization, where transcript(s) is to be sent
* Number of copies needed
